I need help with my concept map for nursing. My medical diagnosis is Syncope for my patient who passed out during dialysis. She has a broken leg, diabetic, high blood pressure, and is 73 years old. I’m having a hard time finding a nursing diagnosis because it can’t be risk for falls or something easy. I also need help with my concept map for this patient. I need a primary concept and a primary exemplar for her diagnosis of syncope and I’m really confused.
