The purpose of this assignment is to give you practical experience responding to Medicare/Medicaid issue identified from a surveyor. Scenario When nursing homes are surveyed by the Ohio Department of Health (ODH), ODH will determine if the facility has failed to meet regulatory standards. Failure to meet these standards will result in a citation which is listed on a survey form called the “2567”. (Just the name of the form). Surveyors are given information and guidance on each citation in a document called the State Operations Manual. Appendix PP is the section of the State Operations Manual which governs surveys for skilled nursing facilities. I have also attached a rubric of the Scope and Severity of the Ciations, to help you uderstand the level at which the citation is cited. (e.g., SS=D). If a Facility is cited, they will have to prepare a Plan of Correction (POC). The reason that the 2567 is arranged in columns is because in the “old days” the POC was inserted and typed into the second column, directly across from the citation. A POC is not a time for argument. The facility, even if it does not agree with the citation, must prepare and document a plan that describes what actions the facility will take to correct the deficiency and specifies the date by which that deficiency will be corrected. POCs generally include the following: Demonstrate that the resident is protected. If it involves a resident, indicate that immediately upon discovery of the issue, the resident was attended to. If the issue could affect other residents, make sure those residents are safe. Determine how this situation can be remedied — do we need education? If so, who needs to be educated? Does someone need to be fired? Does someone need to be hired? How will you monitor compliance? Will you check on with those employees who had to be educated? How frequently?
