nulation tion 0:00/0:44 Liam Natto is a 27-year-old client who was admitted 2 days ago following a work-related injury with multiple perforations to his abdomen and internal organs. The client had surgery that resulted in a bowel resection and has a temporary colostomy. The client was in the intensive care unit (ICU) following surgery but is being transferred to the medical surgical unit. Report from the nurse in the ICU states that the client is alert and oriented but becomes easily confused due to pain medication they are receiving. They have a peripherally inserted central catheter (PICC) placed in their right arm. The client has generalized weakness, deconditioning, and is occasionally forgetful. Basic metabolic panel and complete blood count from this morning are pending. The nurse assumes care for the client. Upon assessment, client’s apical pulse is regular, lung sounds diminished in the bases, bowel sounds are hypoactive. The client remains NPO. Abdominal incision is clean, intact, and open to air. The client lost 6 pounds ( 2.72 kg ) since the admission. The PICC dressing is not intact on one side. The client is asking for pain medication, rating the pain in his abdomen as a stabbing pain of 6 on a pain scale of 0 to 10 . The colostomy stoma is beefy red, no leakage noted around the colostomy bag with 100 ml of liquid output. The client has minimal emotional expression during the morning assessment. The nurse is asking the client questions regarding their recovery. The client states they don’t remember much from the intensive care unit as they were on many pain medications and sleeping a lot. At the end of the assessment, Liam states, “Leave me alone. I have a lot on my mind and need to think about my future.” Vital signs are BP 98/60, Pulse 102, Respirations 22, Temperature 37.3
