Please help with a nursing care plan for a psychiatric pt. Patient’s medical dx ids depression. patient was hospitalized for 6 days and requested to come to inpatient voluntarily because unable to function. Patien’s daughter reported that at home the pt isolates herself and is declinign. When asked, pt stated she sometimes doesn’t think that life is worth living and when at home she has thoughts of suicide by taking pills, but not currently while in psych…the nursing dx i have is risk for injury related to suicidal ideation…the care plan requires 3 sets of : Smart goals,STG,LTG,3 interventions, and outcome…please assist.
